· Current methods for detecting pancreas allograft rejection are limited, necessitating new non-invasive tools.
· This study analyzed molecular HLA matching data from 238 simultaneous pancreas-kidney (SPK) transplant recipients.
· Of the histocompatibility metrics analyzed (HLA mismatches, eplets, Snow, PIRCHE), only PIRCHE-II scores derived from donor HLA class II were significantly associated with clinical events and rejection, primarily pancreas rejection.
· These findings suggest that incorporating PIRCHE scores for class II HLA mismatches could improve risk assessment and outcomes in SPK transplantation.
